This form is for withdrawing a team from the 2023 Nationals. All withdrawal requests must be made through this form. A Volleyball Canada representative will follow up with you to confirm the withdrawal.

Refund Policy

The registration fees will be refunded, less a $350 administration fee & system fees if this form is submitted prior to 11:59PM April 7th, 2023. Any team that cancels their registration after that date will forfeit their entire registration amount.
