To request an athlete age category exemption for participation at 2022 National Championships, fill out the form below. All age category exemptions will be managed by the Athlete and Coach Pathway Committee. All requests MUST be submitted by the coach and/or club contact.

Deadline for Submission: March 15, 2022
Incomplete forms will not be processed.
